WBO Medical Forms

Your place on the trip you choose is confirmed when we receive all forms, filled out and signed, and your full tuition payment.

This medical form is a particularly important way we ensure a safe experience for you. The physician's examination (if applicable) must take place within 12 months prior to the course.

If we have any question about your capability to complete the course, we will call and discuss it with you and/or your physician. If we think you should not participate in the course, we will refund all tuition payments made to Wilderness Based Orientation (WBO).
