We have provided a Verification Request Form (pdf) with all information needed to request verification of enrollment. If you do not use this form, please include the following in your written request:
Current legal name and all previous names
Student Number
Date of Birth
Semester(s) to be verified
Anticipated date of graduation
Where we should send the verification when completed
If the verification is for an insurance company, you must also include:
Policy Holder's Name
Policy Holder's insurance identification number
Company Name
