Student Insurance

NOTES: To obtain evidence of a student's enrollment for the purpose of filing proof of student status with your private health insurance company, contact the Registrar's office, 970-943-2047.

Insurance for international students is handled by the Student Affairs office, 970-943-2090.

Please be aware that this plan cannot be used as primary insurance for student athletes.

Participation in this insurance is VOLUNTARY and enrollment is not passive nor covered by any student fees; you must actively sign up in order to be covered. Premiums are paid directly to Summit America, not WSC.

Student Injury & Sickness Insurance Plan

A supplemental accident and sickness insurance plan for up to $25,000 is available for eligible students attending Western State College. All students are strongly encouraged to have medical insurance coverage as unexpected injuries or illnesses could inflict financial hardship on you and your family.
The plan offered is with Summit America Insurance Services and is underwritten by US Fire Insurance Company. Please check your family insurance plan as there may be limitations regarding student status and age. This plan works well as a supplement to most group plans to relieve the financial hardship a student and their family may experience in case of illness or injury. To view or print out a copy:

Student Health Center

The deductibles under this insurance plan will be waived if students use the Student Health Center located on campus for each injury, illness or referral.


Eligibility

All registered students taking five (5) or more credit hours are eligible to participate in the plan on a voluntary basis. Spouse and children of eligible students may be insured under this program at additional premium. THIS PLAN WILL NOT MEET THE COVERAGE REQUIREMENTS FOR COMPETITIVE ATHLETICS.

Plan information/enrollment packets will be mailed out around the last week of July 2008 to students who are registered by June 30th. Packets will also be available in the Human Resources office in 209 Taylor Hall and at the Cashier's window in 207 Taylor Hall; or a packet can be mailed to you by calling 970-943-3140 with your name and address; or email us at cspritzer@western.edu. The new plan will be underwritten by National Union Fire Insurance Company of Pittsburgh, PA with Summit America Insurance Services providing 3rd party claims administration. the contact information will remain the same--Summit America's information.

Effective date

Coverage will be effective the date the correct premium is received by the Company or the effective date of the coverage period, whichever is later. An effort will be made to send premium renewal notices if paying by installments, but it is the student's responsibility for timely renewal payments whether or not a renewal notice is received. Your canceled check or credit card billing is your only receipt and notification of coverage.

Premiums

Premiums cannot be prorated for enrollment mid-way through the term. Refunds of paid premiums are allowed only upon entry into the armed forces.


How to enroll

Insurance enrollment packets are available in the Human Resources office, Room 209 Taylor Hall, or mailed to registered students' home addresses in late July/early August each year. Payment and enrollment materials are sent directly to the Student Resources, not to Western State College. USE YOUR 6-DIGIT WSC STUDENT ID NUMBER ON THE FORM WHERE IT ASKS FOR "School ID # 0 0 0_ _ _ _ _ _"; you are no longer required to give out your social security number.


Summary of benefits

The policy provides benefits for the usual, reasonable and customary (URC) charges incurred by an Insured Person for loss due to a covered injury or sickness up to the maximum benefit. Specifics of benefits, exclusions and limitaitons are more fully described and defined in the enrollment packet. Details can be viewed at the website or in the brochure.

Claim procedure

In the event of Injury or Sickness, the student should report to the Student Health Center for treatment or referral. When the Student Health Center is closed, report to Gunnison Family medical Center. When not in school, students should report to their physician or hospital.

Claim procedures are outlined on the website & in the procedure. Claims must be submitted within 30 days of service. Claim status can be monitored online. For claim inquiries, please contact the plan administrator at 1-877-246-6997.
