This page will link you to forms needed to apply for student teaching, and information about and forms used during student teaching.

A complete application for student teaching consists of each of the following parts:

You will need to follow the Student Teacher Application Timeline and Due Dates- Word.

The Oath of Agreement and Preliminary Information Sheet are completed at the first mandatory meeting in preparation for student teaching. This meeting takes place within the first month of the semester prior to the semester in which an education student intends to student teach. For example, if an education student intends to student teach in the fall of 2004, then the First Mandatory Meeting for this education student takes place in the first month of the spring of 2004 semester.

  • Oath of Agreement for Student Teaching- Word or PDF.

Items 1 through 4 below must be submitted in electronic formatItems 1-4 and the transcripts must be turned in by the student teaching application due dates each semester. Students must pass the appropriate content area exam BEFORE student teaching. The licensure exams are listed below for each content area:

  1. Application for Student Teaching Cover Sheet- Word or PDF (completed and signed by appropriate professors)
  2. Resume (professional in format and appearance),
  3. Teaching Philosophy Statement (well written),
  4. Academic Data Sheet - Elementary- Word or PDF, Secondary- Word or PDF, K-12- Word or Pdf (appropriate form for the licensure being sought)
  • Transcripts (include transcripts from all institutions attended),
  • Content Area PLACE or Praxis Exam score reported with "pass" score noted (if seeking special education licensure, also the Special Education State Licensure Exam)

    NOTE: You must take the Praxis II exam if you are planning on student teaching in the spring 2004 in the following content areas - Elementary, Science, Social Studies, Math, English.

Students seeking licensure in the following areas have the choice to take the Praxis II or PLACE exams.

Students seeking licensure in the following areas continue to take the PLACE exam. Students seeking licensure in Elementary, English, Mathematics, Science, or Social Studies also have the option to take the PLACE exam.
