New Hire Onboarding

“ON-BOARDING”

Welcome to Western!  This page is to assist a newly hired employee in completing the initial documents necessary to get you
set up and ready.

Upon Acceptance of Hire:

You will need to schedule an orientation on or before your 3rd day of employment.  Please contact the Benefits Administrator
to set up a meeting time:

Temporary or other non-benefited hires will only need to schedule about 20-30 minutes.  Benefited hires will need a couple
hours and may be scheduled in groups.

Before your start date, all new hires should print out and complete the following forms which will allow us to get you set up
with access for your first day:

These forms allow employees to self-identify whether or not they have a disability and whether or not they are protected veterans.

If you choose to complete the forms below, the data we collect will help us produce statistics and will not be used against you in any way. You may return completed forms to the HR office.

What You Need to Bring With You to the HR Orientation:

If you want direct deposit, please bring a canceled check.  Names, birth dates, social security numbers, & addresses of any
insurance or life beneficiaries will help you to fill out forms.

ALL EMPLOYEES are required to complete a Form I-9 no later than their first day of work and within 3 days present original
documents for inspection and copying which prove their 1) identify and 2) work eligibility.  You may choose to present any
document from “List A” below which satisfies both requirements, or a document from “List B” for identity along with a document
from “List C” for work eligibility.  DO NOT MAIL OR SEND SCANNED COPIES as these documents must be physically examined
at the time of your benefits orientation.

If you intend to add family members to your insurance, you will need documents verifying their eligibility for participation.  It is advisable to begin collecting these documents before you attend your new employee orientation session.  Here is the list of acceptable dependent verification documents:
