Information regarding work related injuries or illness

Filing a Report

If an employee sustains an injury while on the job, he/she must immediately notify his/her supervisor of the incident.

The employee must complete the First Report of Injury form (provide detailed answers to ALL questions on the first page) & forward to the Human Resoruces office immediately.

If medical treatment is necessary, all employees must obtain treatment of work-related injuries and illnesses from :

If neither designated physician is available, other doctors at the above-named clinics may treat you. 

In the event of a life threatening or limb threatening emergency, the insured employee should proceed to the nearest emergency medical facility.

In addition, follow-up care must be provided by one of the medical providers designated above.

If an employee is treated by an unauthorized medical provider, the employee may be responsible for the cost of such medical treatment.

Work-related injuries must be reported to a supervisor and a form completed and filed with the Human Resources office immediately, but at a minimum within four (4) days of the injury.

 

Work Status

Each time an employee sees a medical provider for treatment or follow-up related to a work related injury or illness, the employee should receive a Physician's Report from the medical provider.  It is useful for the employee to take the form with them and hand carry the completed report back to the HR office.

 

Insurance
