Annual performance evaluations for classified staff for the period of April 1, 2012 through March 31, 2013 are due in the Human Resources office on Tuesday, April 30, 2013 in accordance with Western's plan (see link above). The original copy of the planning form will be sent out to supervisors on April 1st, along with a blank of the new form for next year's plan. A copy of the Performance Management Guide is available. Classified employee evaluations or plans not received in Human Resources by May 1st, will be referred to the next level supervisor for completion.

Exempt Administrators
Annual performance evaluations for administrators for the period of March 1, 2012 through February 28, 2013 will be due in the Human Resources office on Friday, March 29, 2013 in accordance with our Handbook. The original copy of your "Professional Goals Statement for 2012-13" and "Description of Major Responsibilities" will not be re-routed to you. If you cannot find your copy & need a photocopy, please contact Cheelone at 943-2003.
The employee needs to complete a Summary of Achievements, as a self assessment, and forward to his/her supervisor. The supervisor will meet with the employee and complete the Summative Evaluation.
Instructions for Administrator Evaluations are also available to assist you in completing these forms.
Both original forms are then sent to Human Resources office where they will be attached to the original of the Professional Goals Statement and Description of Major Responsibilities which were prepared last year.
Next prepare a PLAN for the period of March 1, 2013 through February 28, 2014. Complete both the Description of Major Responsibilities and Professional Goals Statement and forward to the Human Resources office.
